A Brief History Of Antibiotic Resistance: How A Medical Miracle Turned Into The Biggest Public Health Danger Of Our Time

1955. As Fleming had predicted, resistance to penicillin gradually built up due to the accessibility of the drug. By 1955, many countries had attempted to slow this resistance by limiting penicillin use to prescription only, but it was too little too late: many bacterial strains had already defeated the antibiotic, including staphylococci.

1960. In an attempt to defeat penicillin-resistant strains, scientists developed methicillin, a different antibiotic in the penicillin class that could work against resistance. But within a year, bacterial strains developed resistance to methicillin too — eventually called MRSA , methicillin-resistant Staphylococcus aureus , or S. aureus . Now, MRSA can resist most antibiotics, and infections are common in hospitals — making it one of the biggest forerunners of multiple-drug resistant (MDR) bacteria.

2012. As more researchers began working on the impending antibiotic-resistant epidemic, they had to tackle the classification of multidrug-resistant bacteria, which were multiplying by the minute. In a 2012 study, a team of scientists proposed adding the terms extensively drug-resistant (XDR) and pandrug-resistant (PDR) to multidrug-resistant (MDR) bacteria to better help them classify and potentially defeat these superbugs. It was the first time that researchers had a unified set of definitions for MDR bacteria to better understand them.

The danger of the situation is mainly in its complexity, Rustav Aminov writes in a 2010 report on antibiotic resistance: “It is not a single grand challenge; it is rather a complex problem requiring concerted efforts of microbiologists, ecologists, health care specialists, educationalists, policy makers, legislative bodies, agricultural and pharmaceutical industry workers, and the public to deal with. In fact, this should be of everyone's concern, because, in the end, there is always a probability for any of us at some stage to get infected with a pathogen that is resistant to antibiotic treatment.”

Understand the threat of antibiotic resistance

Almost half of Americans don't understand threat of antibiotic resistance

When it comes to the growing threat of superbugs, far too many Americans just don’t get it. That’s according to a poll in June of 1,006 adults by Consumer Reports National Research Center that showed 41 percent of people hadn’t even heard of antibiotic resistance.

When asked pointed questions about the risks and benefits of using antibiotics, responses were mixed. While a majority of Americans strongly agreed that taking an antibiotic unnecessarily can make that drug less effective for you in the future, fewer people appear to understand the bigger picture. Roughly just half (52 percent) strongly agreed that bacteria that are resistant to multiple antibiotics, so-called “superbugs,” are a major public health problem today. Forty-nine percent strongly agreed that taking an antibiotic you don’t need makes that drug less effective at treating illnesses in other members of the community in the future. Overall, less than half (45 percent) said they’re somewhat or very concerned that this problem might affect their family.




An equally worrisome finding: More than a quarter (27 percent) of people said they think antibiotics are effective in treating colds and the flu (they aren’t—colds and flu are infections from viruses, not bacteria).


In fact, a second poll we conducted in June of 223 people who received a new antibiotic prescription in the past year showed Americans are being prescribed antibiotics inappropriately for illnesses against which the drugs either don't work or may not be the best treatment, such as sinus infections (12 percent), as a precaution after medical or dental surgery (11 percent), coughs or colds (8 percent), and abnormal urinary symptoms (7 percent).

Misconceptions about antibiotics can lead to misuse. And misuse can result in antibiotic-resistant bacteria, which now sicken at least 2.25 million people each year—and kill 37,000. One way you can help the overall problem of bacterial resistance is by not insisting on antibiotics from your doctor for viral infections. And even when you do have a bacterial infection, if it’s mild, ask your doctor about fighting it off on your own without resorting to the use of an antibiotic. Case in point: sinusitis and bronchitis are usually viral. And even when bacteria are the cause, mild sinus and bronchial infections often clear up in about a week on their own. Also many older people are routinely given antibiotics for findings of urinary bacteria, even though they are lacking symptoms of a urinary tract infection. Such inappropriate use of antibiotics can do more harm than good.



Other steps you can take to help keep you, your family, and everyone else safe from superbugs include: 
  • Requesting targeted drugs. If your doctor determines you need an antibiotic, ask for a culture to be done to identify which bacteria is causing the infection. That should lead to prescription of an antibiotic that can specifically treat the identified bacteria, rather than a broad spectrum drug that kills more types of bacterial, unnecessarily, as well. 
  • Avoiding antibiotic topical creams, unless advised by your doctor. Even antibiotics applied to the skin can lead to resistant bacteria. So use over-the-counter ointments containing bacitracin and neomycin only if dirt remains after cleaning with soap and water. 
  • Preventing infections in the first place. That means staying up to date on vaccinations. And it means washing your hands thoroughly and regularly, especially before preparing or eating food, before and after treating a cut or wound, and after using the bathroom, sneezing, coughing, and handling garbage. Plain soap and water are best. Avoid antibacterial hand soaps and cleaners, which may promote resistance. 
  • Getting rid of leftover antibiotics. More than one out of four people (28 percent) in our poll this past June said they have leftover antibiotics at home. To avoid misuse, dispose of unused antibiotics safely, either by returning them to the pharmacy or mixing them with coffee grounds or cat litter and tossing the mixture in the trash.

Representatives of some 150 organizations, including Consumer Reports, gathered at the White House on June 2, 2015 for a forum on how to rein in the rampant and potentially deadly overuse of antibiotics, from doctor's offices to farms. Consumer Reports President and CEO Marta Tellado reports back from the event.  

Q. Why should consumers care about antibiotic-resistant bacteria?

A. This is the public health crisis of our generation. What we know is that the more antibiotics we use, the more we lose the ability of those drugs to fight bacteria. And what we know right now is that we’re breeding superbugs that are resistant to antibiotics. We have two million people that are affected by bacteria that are resistant to antibiotics, and 23,000 people die as result of the declining efficacy of these vital drugs.

Q. What did the White House Forum accomplish?

A. I was encouraged by some of the calls for more transparency around data and infection rates, especially in hospitals and nursing homes. I think consumers really need to know what those infection rates are. And so that was a very positive development.

Q. What should consumers do if they’re concerned about this problem?

A. Well, I think you have to have a conversation with your physician when you’re prescribed antibiotics. Also, be cautious. A lot of antibiotics are prescribed for the common cold and sore throats and ear infections, which are viral, and antibiotics aren’t useful for those kinds of infections.

Q. What is Consumer Reports doing about this important issue?

A. Well, we’re going to launch a three-part investigative series in Consumer Reports magazine that really looks at the overuse of antibiotics and the misuse of antibiotics. And that has spurred a rise in infection rates in hospitals. And we’re also going to look at the use of antibiotics in the meat supply.

Why you shouldn't ask your doctor for antibiotics
Consumer Reports joins with doctors and hospitals to stop misuse of the drugs

Did you ever ask your doctor for antibiotics to treat a cold? Or the flu? Or what about your child’s painful ear? And did your doctor ever say “Sure! Here you go!” Well, you probably shouldn’t have asked—and your doctor almost certainly should have said no.

Why? Because viruses usually cause those infections, not bacteria—and antibiotics don’t work against viruses. (Have you wondered why you did or didn't receive a prescription for antiobiotics?

And using antibiotics when you don’t need them is not only a waste of money, but can cause side effects including everything from diarrhea to nausea, vomiting, and serious allergic reactions. Even worse, the unnecessary use of antibiotics can breed bacteria that are resistant to antibiotics, making the drugs less effective when you really do need them. The Centers for Disease Control and Prevention estimates that up to half of all the antibiotics used in this country are prescribed for the wrong reasons.

To help combat this problem, Consumer Reports is partnering with the ABIM Foundation (with funding from the Robert Wood Johnson Foundation) to work with seven health care groups across the country to get doctors to cut their inappropriate prescribing of antibiotics for upper respiratory infections–like the common cold and the flu—by at least 20 percent over the next three years. Each of the seven groups includes representatives from organizations such as hospitals, clinics, employers, community groups, and medical societies, like the California chapter of the American College of Physicians.

One of the groups, based in Wisconsin, plans to launch a public awareness campaign with TV, radio, and newspaper ads to let people know about overuse of antibiotics–and what alternatives there are for people with runny nose and other symptoms from the common cold.

Other groups, based in both the East and West coasts, will work with health care providers so that when a doctor and patient discuss antibiotics, the doctor can print out information from Consumer Reports explaining why antibiotics won’t work for their infection – and what to do instead. Printouts of those brochures are available in English and in Spanish.

The groups will also work on reducing other examples of medical overuse that can lead to harm, including MRIs for new low-back pain with no other major symptoms, and benzodiazepines such as Valium and Xanax, drugs frequently used to combat anxiety and insomnia in older adults.
